Tracking Sheet Issues and Omni Holiday Closure

Tracking Sheets:

Omni has identified several issues with the weekly tracking sheets that have resulted in completed surveys not showing up. We sincerely apologize for any inconvenience this has caused. We are working diligently to resolve this issue and will send out another email once it is fixed and new tracking sheets are available. Due to our upcoming company holiday closure, tracking sheet creation will be paused until the week of 1/5/26.

In the meantime, we recommend always entering your email at the end of the survey to receive a confirmation that the survey was submitted successfully. The 6-month and Annual assessment windows are still accurate in the latest tracking sheet dated 12/15/25 and can be referred to as needed.

Omni staff will be available through Tuesday, 12/23, to respond to any questions via email at SORSupport@omni.org.

Omni Holiday Closure:

Omni will be closed from December 24th through January 1st and reopening on Friday, January 2nd. We are taking some time to enjoy the end of the year and re-energize for 2026. We hope you can find some time to do the same!

Our entire team will be unavailable for project-related support during these dates. Any emails or voicemail messages will be reviewed and responded to as soon as possible starting January 2nd.
