Corrected Tracking Sheets Now Available!
In December, Omni identified several issues with the weekly tracking sheets that were resulting in completed surveys not showing up. As of Monday afternoon, 1/5, these issues have been resolved. Corrected tracking sheets are now available in your CSB/Agency’s Dropbox folder.
As a reminder, we recommend always entering your email at the end of the survey to receive a confirmation that the survey was submitted to us successfully.
SUPRT Record Closeout Reminder for GPRA Clients
We wanted to provide a brief reminder about which SUPRT surveys should be entered for clients who were still active at the end of SOR IV Year 1 (i.e., they had not completed a GPRA discharge).
When an active GPRA client stops receiving SOR-funded services, a Record Closeout should be entered to close that “episode of care”. Once an episode of care is closed, no other surveys should be entered for the timeline associated with that GPRA Intake. If the client returns at a later time (i.e., they begin a new episode of care), then a Baseline SUPRT should be completed. The windows for the 6-month and Annual follow-up assessments will be calculated in accordance with the new Baseline.
Note: Do not complete a new Baseline for a client without closing out the previous episode of care. For active GPRA clients only, a 6-month or Annual SUPRT survey should be entered without entering a Baseline SUPRT. If you are receiving errors attempting to enter a 6-month or Annual survey for an active GPRA client, please reach out to us. In this scenario, don’t enter a Baseline survey as this will indicate the start of a new episode of care for the client and any surveys entered after that Baseline will not be associated with their GPRA surveys.
