Reminder: Submit Follow-up Assessments in Open Windows – SOR IV Year 2 Closing Soon

As SOR IV Year 2 is quickly drawing to a close on September 30, 2026, we would like to remind you all to please submit follow-up assessments for clients with open survey windows.

One can identify clients that have open windows by checking the weekly tracking sheet. The color-coded key at the top of each tracking sheet will let you know which cases are currently open.

We've noticed that a number of CSBs/agencies currently have open windows, and we want to encourage you all to submit any surveys that fall into that category to help wrap up the year. Making sure this data is complete will assure a more accurate annual report, showcasing the full scope of the work you all did and what the funding accomplished.

Virginia’s State Opioid Response (SOR) grant is awarded at the discretion of DBHDS and SAMHSA. All recipients of SOR funding supports evidence-based prevention, treatment and recovery services to address opioid and stimulant misuse and use disorders, including for cocaine and methamphetamine. The SOR grant is not intended to be utilized as a long-term or permanent funding source. All SOR proposals must include a realistic plan for financial sustainability beyond the grant period.
